Urban insurgency has been used with increasing frequency and effectiveness in many areas of the developed and less developed world during the past decade. In Latin America, this trend toward expanded urban guerrilla warfare has been most pronounced in Brazil, Uruguay, and Argentina. In the three nations, revolutionary forces have rejected completely the concept of the primacy of guerrilla activities based in the countryside, a theory adapted to the Latin American environment by Cuba's Ernesto “Che” Guevara and French Marxist Régis Debray. Instead, attention has been focused on organizing and developing guerrilla and terrorist operations in such population centers as Rio de Janeiro, São Paulo, Montevideo, Buenos Aires, Rosario, and Córdoba. Objective:
The aim of the study was to assess occupational health effects 1 month after responding to a natural gas pipeline explosion.
Methods:First responders to a pipeline explosion in Kentucky were interviewed about pre- and post-response health symptoms, post-response health care, and physical exertion and personal protective equipment (PPE) use during the response. Logistic regression was used to examine associations between several risk factors and development of post-response symptoms.
Results:Among 173 first responders involved, 105 (firefighters [58%], emergency medical services [19%], law enforcement [10%], and others [12%]) were interviewed. Half (53%) reported at least 1 new or worsening symptom, including upper respiratory symptoms (39%), headache (18%), eye irritation (17%), and lower respiratory symptoms (16%). The majority (79%) of symptomatic responders did not seek post-response care. Compared with light-exertion responders, hard-exertion responders (48%) had significantly greater odds of upper respiratory symptoms (aOR: 2.99, 95% CI: 1.25–7.50). Forty-four percent of responders and 77% of non-firefighter responders reported not using any PPE.
Conclusions:Upper respiratory symptoms were common among first responders of a natural gas pipeline explosion and associated with hard-exertion activity. Emergency managers should ensure responders are trained in, equipped with, and properly use PPE during these incidents and encourage responders to seek post-response health care when needed.

Objective:
To describe epidemiologic and genomic characteristics of a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) outbreak in a large skilled-nursing facility (SNF), and the strategies that controlled transmission.
Design, setting, and participants:This cohort study was conducted during March 22–May 4, 2020, among all staff and residents at a 780-bed SNF in San Francisco, California.
Methods:Contact tracing and symptom screening guided targeted testing of staff and residents; respiratory specimens were also collected through serial point prevalence surveys (PPSs) in units with confirmed cases. Cases were confirmed by real-time reverse transcription–polymerase chain reaction testing for SARS-CoV-2, and whole-genome sequencing (WGS) was used to characterize viral isolate lineages and relatedness. Infection prevention and control (IPC) interventions included restricting from work any staff who had close contact with a confirmed case; restricting movement between units; implementing surgical face masking facility-wide; and the use of recommended PPE (ie, isolation gown, gloves, N95 respirator and eye protection) for clinical interactions in units with confirmed cases.
Results:Of 725 staff and residents tested through targeted testing and serial PPSs, 21 (3%) were SARS-CoV-2 positive: 16 (76%) staff and 5 (24%) residents. Fifteen cases (71%) were linked to a single unit. Targeted testing identified 17 cases (81%), and PPSs identified 4 cases (19%). Most cases (71%) were identified before IPC interventions could be implemented. WGS was performed on SARS-CoV-2 isolates from 4 staff and 4 residents: 5 were of Santa Clara County lineage and the 3 others were distinct lineages.
Conclusions:Early implementation of targeted testing, serial PPSs, and multimodal IPC interventions limited SARS-CoV-2 transmission within the SNF.

Summary
Abstract. We trace the emergence of unsolvable problems in algebra and topology from the unsolvable halting problem for Turing machines.
§1. Introduction. Mathematicians have always been interested in being able to calculate with or about the things they study. For instance early developers of number theory and the calculus apparently did extensive calculations. By the early 1900s a number of problems were introduced asking for general algorithms to do certain calculations. In particular the tenth problem on Hilbert's influential list asked for an algorithm to determine whether an integer polynomial in several variables has an integer solution.
The introduction by Poincaré of the fundamental group as an invariant of a topological space which can often be finitely described by generators and relations led to Dehn's formulation of the word and isomorphism problem for groups. To make use of such group invariants we naturally want to calculate them and determine their properties. It turns out many of these problems do not have algorithmic solutions and we will trace the history and some of the ideas involved in showing these natural mathematical problems are unsolvable.
In the 1930s several definitions of computable functions emerged together with the formulation of the Church-Turing Thesis that these definitions captured intuitive notions of computability. Church and independently Turing showed that there is no algorithm to determine which formulas of first-order logic are valid, that is, the Entscheidungsproblem is unsolvable.

Objective.
Describe central line-associated bloodstream infection (CLA-BSI) epidemiology in pediatric intensive care units (PICUs).
Design.Descriptive study (29 PICUs); cohort study (18 PICUs).
Setting.PICUs in a national improvement collaborative.
Patients/Participants.Patients admitted October 2006 to December 2007 with 1 or more central lines.
Methods.CLA-BSIs were prospectively identified using the National Healthcare Safety Network definition and then readjudicated using the revised 2008 definition. Risk factors for CLA-BSI were examined using age-adjusted, time-varying Cox proportional hazards models.
Results.In the descriptive study, the CLA-BSI incidence was 3.1/1,000 central line-days; readjudication with the revised definition resulted in a 17% decrease. In the cohort study, the readjudicated incidence was 2.0/1,000 central line-days. Ninety-nine percent of patients were CLA-BSI-free through day 7, after which the daily risk of CLA-BSI doubled to 0.27% per day. Compared with patients with respiratory diagnoses (most prevalent category), CLA-BSI risk was higher in patients with gastrointestinal diagnoses (hazard ratio [HR], 2.7 [95% confidence interval {CI}, 1.43-5.16]; P<.002) and oncologic diagnoses (HR, 2.6 [CI, 1.06-6.45]; P=.037). Among all patients, including those with more than 1 central line, CLA-BSI risk was lower among patients with a central line inserted in the jugular vein (HR, 0.43 [CI, 0.30-0.95]; P<.03).
Conclusions.The 2008 CLA-BSI definition change decreased the measured incidence. The daily CLA-BSI risk was very low in patients during the first 7 days of catheterization but doubled thereafter. The risk of CLA-BSI was lower in patients with lines inserted in the jugular vein and higher in patients with gastrointestinal and oncologic diagnoses. These patients are target populations for additional study and intervention.

Collaboration is used by the US National Security Council as a means to integrate inter-federal government agencies during planning and execution of common goals towards unified, national security. The concept of collaboration has benefits in the healthcare system by building trust, sharing resources, and reducing costs. The current terrorist threats have made collaborative medical training between military and civilian agencies crucial.
This review summarizes the long and rich history of collaboration between civilians and the military in various countries and provides support for the continuation and improvement of collaborative efforts. Through collaboration, advances in the treatment of injuries have been realized, deaths have been reduced, and significant strides in the betterment of the Emergency Medical System have been achieved. This review promotes collaborative medical training between military and civilian medical professionals and provides recommendations for the future based on medical collaboration.

Background
Comorbid anxiety is common in depressive disorders in both middle and late life, and it affects response to antidepressant treatment.
AimsTo examine whether anxiety symptoms predict acute and maintenance (2 years) treatment response in late-life depression.
MethodData were drawn from a randomised double-blind study of pharmacotherapy and interpersonal psychotherapy for patients age 70 years and over with major depression. Anxiety symptoms were measured using the Brief Symptom Inventory. Survival analysis tested the effect of pre-treatment anxiety on response and recurrence.
ResultsPatients with greater pre-treatment anxiety took longer to respond to treatment and had higher rates of recurrence. Actuarial recurrence rates were 29% (pharmacotherapy, lower anxiety), 58% (pharmacotherapy, higher anxiety), 54% (placebo, lower anxiety) and 81% (placebo, higher anxiety).
ConclusionsImproved identification and management of anxiety in late-life depression are needed to achieve response and stabilise recovery.

We describe a general technique for embedding certain amalgamated products into direct products. This technique provides us with a way of constructing a host of finitely presented subgroups of automatic groups which are not even asynchronously automatic. We can also arrange that such subgroups satisfy, at best, an exponential isoperimetric inequality.

Compressive yield stresses have been measured for pastes (0.35 ≤ w/c ≤ 0.50) of portland cement, calcium aluminate cement, and weakly and strongly flocculated alumina (Φ0 = 0.20) using the centrifuge sediment height technique. Equilibrium sediment heights are reached quickly, allowing all measurements to be taken during the cement's induction period. The compressive behavior showed little dependence on the compressive history. Compressive yield stress was, however, dependent upon initial volume fraction, decreasing as the initial volume fraction increases. This behavior was observed in both the cements and alumina suspensions, implying that strong dependencies on initial structure may be a general property of the compressive behavior of flocculated suspensions.

In the fossil record of the Paleozoic, epifauna are typically shelly, whereas infauna are typically soft-bodied; the two groups have tended to be studied in isolation from one another, and commonly by different specialists. We believe that the understanding of patterns of distribution and evolution of epifauna cannot proceed in the absence of data on infauna. Over the past decade, several workers have attempted to combine information about the two groups into models of community evolution.
